New Patient Registration
New patients living within the Practice catchment boundary can apply to be registered with the practice. The overall boundary area of the Practice is greater Luton. The core area covers the Biscot/Bury Park districts.
Enquiries regarding registration can be made in person, over the telephone or by letter or electronic means such as email.
Persons wishing to register must fill out the application form provided (parents should do this for younger children) and submit acceptable proof of identity such as birth certificate, passport, driving licence, marriage certificate, etc. In most cases this must include a photograph.
The application form has space for including details of the previous medical Practice with which the person was registered and these must be provided (if appropriate).
Although the transfer of medical notes from one surgery to another can take several weeks, our registration process is normally completed within two days, after which the patient is officially accepted onto the Practice list, can book a consultation and is eligible to receive treatment and advice.
New patients are expected to attend soon after registration for a routine preliminary health check at which the patient’s medical history etc. will be discussed and any significant additions or changes recorded.
Change of Address
Patients should notify the surgery as soon as possible if they change their address or telephone number. This is not just for the sake of accurate record-keeping; in an emergency it could be vital.
